The characteristics of early period of adaptation in different clinical variants of intrauterine growth restriction

Abstract :

Intrauterine Growth Restriction (IUGR) may lead to a change in the function of organs and systems due to violation of postnatal adaptation and a negative impact on child development at later stages of ontogeny. The aim of our study was to determine the characteristics of the adaptation period in newborns intrauterine growth retardation, depending on the clinical variant. We observed 315 newborns with IUGR born at term 37-41 weeks of pregnancy. Of these, 172 children were with asymmetric variant of IUGR, 143 children - with symmetric variant of IUGR. It was found that in newborns with symmetric variant of IUGR, Apgar scores at 1st and 5th minute there was a significant difference with asymmetric IUGR. Thus, in the first minutes of life in children with symmetric variant Apgar score was significantly lower 6.78�?±0.04 than compared with asymmetric IUGR 6.92�?±0.02 (p<0.001); in the 5thminute when the symmetric variant â�?�? 7.50�?±0.05, asymmetric variant â�?�? 7.67�?±0.04 points (p2<0.01). Children with asymmetric IUGR for the first time breastfeeding have significantly earlier, on 1.55�?±0.06 day, whereas in the symmetric IUGR - on 2.48�?±0.13 day (p<0.001). In newborns with IUGR neonatal jaundice occurred in 23.8�?±2.4% cases: In newborns with asymmetric IUGR jaundice was observed in 2 times less 15.1�?±2.7%, than in children with symmetric variant â�?�? 34.3�?±4.0% (p<0.001). Duration of jaundice in newborns with asymmetric IUGR was 6.8�?±1.0 days, with a symmetric â�?�? 6.6�?±0.5 days.Thus, during the early neonatal period in children with IUGR symmetric variant is characterized by lower compensatory-adaptive reactions.

Garayeva S Z has completed her PhD at the age 26 year from Azerbaijan Medical University. She is Associate Professor in Department of Children Diseases in Azerbaijan Medical University. She has published more than 30 papers in reputed journals and participated in many conferences. She works as a doctor in the department of pathology of newborn of Scientific-Research Institute of Pediatrie in Baku.
