Abstract :

Tetralogy of Fallot incidence is 5-10% of all congenital heart defects (CHD) and is the most common cyanotic CHD. We followed 45 patients from birth to 26 yrs of age since 1988. We have discussed clinical presentations, method of diagnosis, diagnostic work up, and both medical and surgical managements. Also we emphasized post operative follow up and what to expect post operatively for the primary care referring physicians and the parents. The indication of pulmonary valve placement surgery e.g. dilated right ventricle measured by cardiac MR volumetric measurements e.g. 150 ml/msq body surface area even without symptom and widened QRS complex over 180 ms were discussed. In conclusion, we pointed out that the diagnosis could be easily made in prenatal cardiac ultrasound to plan for parental counseling and delivery in a tertiary care center. Complete surgical repair between 3-6 months of age is strongly recommended. This is to preserve the RV myocardial function, decrease the incidence of ventricular tachycardia and sudden death. To avoid trans-annular patch when possible to reduce the risk of Pulmonic valve placement in the future is discussed.
