Relationship Between Migraine-Associated Neuroinflammation and Clinical Response to CGRP-Targeted Therapy

Abstract :

Migraine is a complex neurological disorder involving abnormal sensory processing, trigeminovascular activation, and neu rogenic inflammation. The identification of biomarkers associated with disease severity and therapeutic response remains an important area of migraine research. This study investigated the relationship between inflammatory markers, clinical characteristics, and response to calcitonin gene-related peptide (CGRP)-targeted therapy in patients with migraine. Partic ipants with episodic or chronic migraine were assessed before and after initiation of CGRP-targeted preventive treatment. Clinical parameters included monthly migraine days, headache intensity, duration of attacks, and use of acute medication. Se lected inflammatory and CGRP-related biomarkers were evaluated in relation to treatment response. Patients demonstrating a clinically meaningful reduction in monthly migraine days showed differences in biomarker profiles compared with individ uals with limited therapeutic response. Greater baseline disease burden and selected inflammatory characteristics were also associated with differences in treatment response. These findings support the potential involvement of neuroinflammatory mechanisms in migraine pathophysiology and suggest that biomarker-based approaches may contribute to more individ ualized preventive treatment strategies. Further prospective studies involving larger patient populations are required to establish reliable biomarkers for predicting response to CGRP-targeted therapies.
