Perinatal outcome in patients with antepartum eclampsia

Abstract :

Aims: To study the perinatal outcome in patients with antepartum eclampsia in relation to gestational age. Materials & Methods: A retrospective study including 40 patients attending Mamata hospital with antepartum eclampsia from August 2010 to April 2012. Perinatal outcome in terms of Intra Uterine Death/Stillbirth/Perinatal mortality and morbidity was assessed among these patients. Results: Among the 40 patients studied, 15 (37.5%) had term pregnancies, 23(57.5%) had pregnancies between 28-37 weeks & 2 patients (5%) had gestational age <28 weeks. Among the term pregnancies, 80% were live births & 20% IUDs. 65.22% of pregnancies between 28-37weeks gestation were live births, 17.39% IUDs, 8.69% still births & 8.69% were postnatal deaths. All the pregnancies below 28 weeks gestation were IUDs. Conclusion: Patients with gestational age> 37 weeks had better perinatal outcome in comparison with the other 2 groups. The main cause of perinatal mortality among patients with term pregnancies was IUD (occurred prior to admission). Pregnancies with <28 weeks gestation had 100% mortality rate. Among pregnancies between 28-37 weeks, 8.69% had still births & another 8.69% had postnatal deaths (associated with IUGR and abruptio placentae). Hence better perinatal outcome can be expected with early detection of pre-eclampsia, prevention of eclampsia& earlier admission to hospital. The mode of delivery, induction- delivery interval, associated maternal complications like abruption & fetal complications like IUGR adversely affect the perinatal outcome.
