Abstract :

Acute emergencies in Pediatric age group demand an aggressive approach to minimise the morbidity and mortality. This aggressive approach is needed to utilise the First Golden hour of the critical illness for a better prognosis in the child. The facilities available and the readiness of the personnel attending are critical to achieve this goal. An organised, scientifically validated and protocolised approach is needed for the best outcomes with minimal morbidity and intact neuromuscular survival. To achieve this goal, invasive monitoring facilities, immediate access to blood gases, and immediate imaging facilities are the best bet, but are limited to advanced centres and developed countries. Most of the developing countries and those of the peripheral medical facilities who do not have access to such high end diagnostic luxuries due to resource limitations can give comparable good outcomes with the help of a unique and clinical based scientific approach that is described here. The paradigm shift in the approach to the acute emergencies in children from pathology based to physiological status based helps to identify the physiological derangementswhich are as important as its treatment and correct it initially during the golden hour. It includes a goal directed, core clinical signs based sequence of rapid cardiopulmonary cerebral assessment that can be done in as fast as 60 seconds!! The findings form the components of a Pediatric Assessment Triangle (PAT) which has been modified to help take therapeutic decisions. The innovative use of providing Positive End-Expiratory Pressure (PEEP) with the help of a commonly used anaesthetic circuit has reduced the need of invasive mechanical ventilation in certain conditions during resuscitation and help in faster achievement of therapeutic goals. This protocol based systematic approach, rapid cardiopulmonary cerebral assessment, appropriate intervention, monitoring and appropriate documentation along with a motivated and dynamic team work helps to achieve the goal of Intact Neuromuscular Survival with a smile on the faces of the child, parents and the Resuscitation team as well and goes beyond the usual resuscitation and is the true Joy of Resuscitation!!
