Abstract :

Introduction: Status epilepticus is defined as a single epileptic seizure of >30 minutes duration or a series of epileptic seizures during which function is not regained between ictal events in a 30-minute period .It’s a life threatening condition and can be a complication of central nervous system infections (such us encephalitis), metabolic disease ,electrolyte imbalance ,traumatic brain injury ,drugs intoxication. The usual classification of SE includes focal focal SE with or without impairment of consciousness ,generalized convulsive SE including tonic-clonic ,tonic and clonic and generalized nonconvulsive SE ,including absence SE. Purpose: The purpose is to raise clinical suspicion for this rare complication of the disease in order to achieve the appropriate treatment. Material: A clinical case of a 2,5 year old girl with covid encephalitis is presented Method: A 2,5 year old girl presented to our Emergency Department due to tonic-clonic contractions of the upper and lower limbs wich has started 40 minutes ago. Apositive Sars Cov2 rapid test is reported the day before. This is the first episode of seizures in the second day of the fever. It is the first child of the family without medical history of illness, fully vaccinated . During the clinical examination the patient has GCS: 5/15 ,with tonic-clonic contractions of the upper and lower limbs ,without signs of meningeal irritation ,pupils with constraction ,without the presence of rash. She was febrile ,hemodynamically unstable with electrolyte imbalance (K+ : 1,6) , acidosis (pH:7,09) and high markers of inflammation (ferritin:311,3 ,D- dimmer:1.346). Due to non-resolution of the SE ,the patient was intubated and a brain CT was performed. The CT did not reveal any tumor and the patient immediately transferred to the Paidon Hospital in Athens. Subsequently a brain MRI was performed which showed diffuse cerebral edema as a result of encephalitis following a covid infection. The patient died a few days later. Conclusion: Sars Cov2 encephalitis is a very rare complication and this was the second case in Greece ,in the pediatric community. For this reason ,we should always be ready to deal with even the rarest complication from a ,in the majority of cases, innocent virus.
