Boerhaave syndrome: An unexpected complication of diabetic ketoacidosis

Abstract :

Boerhaave syndrome (BS) is a rare but critical condition characterized by transmural esophageal perforation, often due to forceful vomiting or increased intraesophageal pressure. Differentiating BS from conditions like Mallory-Weiss syndrome is crucial, as clinical presentations vary beyond the classical Mackler triad. Delayed diagnosis significantly increases mortality, reaching 60% with intervention and nearly 100% without treatment. Management ranges from conservative care to surgical intervention, depending on clinical severity and timing. BS is rarely associated with diabetic ketoacidosis (DKA), a serious complication of diabetes often presenting with nausea and vomiting. This report details a 14-yearold diabetic patient who developed BS following severe, recurrent emesis during a DKA episode. Despite initial DKA management, persistent epigastric pain and dyspnea prompted further evaluation, revealing pneumoperitoneum on imaging, confirming BS. This case highlights the potential for severe vomiting in DKA to cause esophageal rupture, emphasizing the need for early recognition and prompt intervention to improve outcomes.

Dr. Haitham Moussa Jarikji is a dedicated Pediatric ER Physician with nearly five years of experience, including three years at Dr. Sulaiman Al Habib Hospital- Qassim, Saudi Arabia, managing critically ill infants and children. He specializes in emergency pediatric care, treating cases such as cardiac arrests, seizures, trauma, and poisoning. He earned his Bachelor’s degree in General Medicine and Pediatrics from Beirut Arab University, completing his internship at Al Zahraa and Hamoud hospitals and a three-year residency at Saida Governmental Hospital. He has worked as a Pediatric Specialist at Rasoul Aazam Hospital, covering Pediatric ED, NICU, and PICU, and as a General Pediatrician in multiple hospitals. Dr. Haitham is skilled in intubation, chest tube insertion, needle decompression, and neonatal procedures, making him a vital asset in pediatric emergency care.
