  Background: Spondylolisthesis is a frequent cause of low back pain but is seldomly associated with infectious causes and multisystem failure. Case presentation: We present a patient who complained of severe low back pain and acute onset of neurological deficits that progressed to cardiovascular collapse and developed into a complex diagnostic and therapeutic scenario requiring intensive care management and surgical intervention.
