Aim: To evaluate the efficacy and safety of trabeculectomy with collagen implant in patients with medically uncontrolled glaucoma. Methods: This is prospective, non-randomized, interventional case series. 76 eyes of 76 patients with medically uncontrolled glaucoma were enrolled. Detailed ocular examination was done in each subject & details were recorded. Conventional fornix based trabeculectomy with subconjunctival collagen (Ologen) implantation was performed in all patients. Preoperative data included age, gender, best corrected visual acuity (BCVA), intraocular pressure (IOP), glaucoma type and number of preoperative glaucoma medications was recorded. Post-operative IOP, number of post-operative glaucoma medications & post-operative complications were recorded. Each patient was followed up for at least 6 months. Results: Mean pre-operated IOP was 34.21�±12.5 with 2.3 number of IOP lowering medications (range 2-3). Post-operative IOP, after 3 month was 13.85�±5.42 mm of Hg (p value 0.060) after 6 month was 16.42�±6.42 mm of Hg (p value 0.056). Mean IOP lowering medications and IOP reduction after 6 months were 0.17 (P=<.001)) and 17.79 mmHg, respectively. Complete success at month 3 was 97.36%, and at the last visit was 88.15%. Post-operative complications within one month were hyphema (n-3), shallow anterior chamber (n-1), and wound leak (n-2). All were improved with conservative management gradually. 3 patients developed cataract within 3 months. Conclusion: Filtration surgery with collagen implantation is a safe and effective treatment to control IOP non responsive to topical anti-glaucoma medications.
