Protracted conflict in Darfur has resulted in large-scale internal displacement and a prolonged weakening of primary healthcare (PHC) systems serving internally displaced populations (IDPs).Persistent insecurity, chronic resource shortages, workforce gaps, and fragmented coordinationbetween governmental and humanitarian actors have compromised service readiness and disease surveillance, contributing to recurrent infectious disease outbreaks and delayed response.
This PhD research aims to systematically assess primary healthcare service readiness and disease surveillance capacity in conflict-affected IDP settings in Darfur, and to develop an evidence-based, context-specific framework for strengthening integrated PHC and surveillance systems. The study will adopt a convergent mixed-methods design. Quantitative components will include facility-level service readiness assessments using an adapted WHO Service Availability and Readiness Assessment (SARA) framework, alongside household surveys examining healthcare access, utilisation patterns, and barriers among IDP communities. Qualitative data will be generated through in-depth interviews with health workers, IDPs, and humanitarian stakeholders to explore lived experiences, perceptions of service quality, and surveillance practices in fragile settings.
Quantitative and qualitative findings will be analysed separately and integrated through triangulation to identify system-level strengths, bottlenecks, and contextual challenges. The PhD is expected to generate original empirical evidence on the performance of PHC and disease surveillance in protracted humanitarian crises and to inform the development of a practical strengthening framework tailored to IDP contexts in Darfur. Findings will contribute to global health systems research and provide actionable insights for policymakers, humanitarian agencies, and public health practitioners working in conflict-affected settings.
