Background: Spontaneous closure of full thickness macular hole (FTMH) is very rare event. Spontaneous closure is seen more commonly in traumatic macular hole (TMH ) but very uncommon in post vitrectomy macular hole. We report a case of full thickness macular (FTMH) with spontaneous closure in previously vitrectomized eye with improvement in final best corrected visual acuity (BCVA). Case presentation: A 26 year old male patient underwent combined Pars Plana Vitrectomy (PPV) and Pars Plana baerveldt shunt insertion and phacoemulsification for traumatic vitreous haemorrhage with cataract and angle recession glaucoma. There was no intraoperative complications. The patient developed full thickness macular hole (FTMH) 2 weeks postoperatively. The FTMH closed spontaneously on 3 week follow up with improvement in best corrected visual acuity Conclusion: Though very rare event, spontaneous closure of macular holes developed after vitrectomy can happen as part of their natural course . Observation in the initial stage is a reasonable management option.
