Comparison of outcomes of DMEK with and without 'S' mark on the donor cornea

Abstract :

Aim: To compare the outcomes of Descemet's membrane endothelial keratoplasty (DMEK) without 'S' mark on the donor cornea with that of 'S' mark on the donor cornea. Methods: Fifty eyes of 45 patients with endothelial dysfunction underwent DMEK. The first 28 had no 'S' mark on the stromal side of the donor cornea while the next 22 had 'S' mark on the stromal side of the Descemet membrane (DM) of the donor cornea made through a 3 mm punch in the donor cornea. The outcome of DMEK in both the groups was compared. Parameters studied included donor corneal age, donor scroll pattern, intra-operative surgical difficulties, post-operative outcomes and complications. Results: In the initial cases performed by the surgeon, donor tissue marking was not performed. The graft orientation was essentially based on the tissue scroll pattern. The mean donor age in first group was 58.3+2.5 and for second group was 62.4+3.6. Of the initial 28 cases where no marking was performed primary graft failure was noted in six cases despite the graft adherence to the stroma, partial graft detachment was seen in three cases. In the later 22 cases, no primary graft failure was seen. Partial graft detachment was seen in one case which was successfully rebubbled. In the older tissues, the double scroll pattern was sometimes misleading and only on the basis of 'S' mark the graft could be oriented in the corrected position. Conclusions: 'S' marking of the DM reduced intraoperative manipulations and improved postoperative outcomes.
