Abstract :

Purpose: Trans-anal procedure, soave, for Hirschsprung Disease (HD) is most commonly performed at early age with good outcome. We come across in our Center�??s experience with late presentation and in all cases single stage Soave had successfully done. We chose to apply these ideas to the primary treatment of HD and describe technical aspects and impact without assisted laparoscopic abdominal approach. Methods: We reviewed our series of HD patients who underwent a trans-anal, assessing for postoperative stricture, anastomotic leak, enterocolitis and results for bowel function. Results: Of 17 patients, 13 had a trans-anal resection, 1 had lost follow up and 2 had a leveling colostomy prior to referral and 1 had ileostomy. The average length of resection was 20±10.5 cm. Mean follow-up was 12.2 months (range 5�??22 months). 7 patients were 3-14 years old, at follow-up were assessed for fecal continence; all (100%) had voluntary bowel movements, constipation (requiring laxatives) not occurred. Of all patients, 13 were male and 4 were female. Conclusion: Our short data review support the fact that a single stage Soave approach and no trans-abdominal dissection is an excellent technique for late presenter Hirschsprung and produce excellent outcomes for fecal continence.

Ubaid Ullah Khan is currently working as a Pediatric Surgeon in Al Hada Armed Forces Hospital, Taif, Saudi Arabia. He has completed his Master Degree from Quide- Azam postgraduate medical college, PIMS, Islamabad in 2010. Then he continued his EBPS in UK from 2013-2014.
