The Effect of Illness Perception on Stigma in Hemodialysis Patients: the Mediating Role of Self-compassion and Healthrelated Hardiness

Abstract :

Objective: This study explored the role of illness perception on stigma in hemodialysis patients and analyzed the mediating role of self-compassion and health-related hardiness. Methods: A cross-sectional study included 271 hemodialysis patients. All participants completed the Sociodemographic and clinical characteristics Questionnaire, The Brief Illness Perception Questionnaire (BIPQ), Self-compassion Scale (SCS), The Health-related Hardiness Scale (HRHS), and Stigma Scale for Chronic Illness (SSCI). The collected data were subjected to correlation analysis, univariate and multifactor analysis, and Amos mediation analysis. Results: The results of the correlation analysis showed that the SSCI scores were positively correlated with BIPQ scores (r=0.454, P<0.001). After adjusting for the confounding factors, negatively correlated with Positive self-compassion (PSC) scores (r =−0.201, P<0.001), positively correlated with negative self-compassion (NSC) scores (r =0.421, P < 0.001), and negatively correlated with health-related hardiness (HRH) scores (r =−0.149, P <0.001). The results of the mediation analysis showed that the parallel mediation model of illness perception to stigma through self-compassion and health-related hardiness yielded an acceptable fit of the data (χ2=649.29, df=235, χ2 /df=2.763, p<0.001, CFI=0.941, IFI=0.941, RMSEA=0.068 [0.061, 0.077]). Conclusion: PSC, NSC, and HRH all play a partially mediating role in the association between illness perception and stigma. Among them, NSC plays a more important mediating role than HRH and greater than PSC. caregivers should assess SC and HRH when caring for hemodialysis patients patients and can use mindful Selfcompassion interventions, paying particular attention to reducing NSC levels and adding patients’ personality traits to the care plan to reduce patients’ sense of stigma and improve mental health.
