Introduction: Periorbital hyperpigmentation (POH) is a complex entity with a multifactorial etiology factors promoting its occurrence can be exogenous as well as endogenous. Dermoscopy of periorbital area may help in finding origin of pigment whether it is due to melanin or due to underlying vasculature. Polarized dermoscopy may be useful in evaluating pigment network and vascular structures, whereas non-polarized dermoscopy was utilized in evaluating superficial skin changes such as scaling and fissuring. Aims and objectives: The study aimed to assess clinical and dermoscopic features of POH and association of dermoscopic features with clinical subtypes of POH. Material and Methods: It was a crossectional hospital based study of 90 patients with POH from the outpatient department, dermatology. Diagnosis of POH was made clinically and the patients were classified according to the classification proposed by Ranu et al. Digital photographs and dermoscopy photographs in both polarized and non-polarized light were taken. Conclusion: Clinically, constitutional type was the commonest presentation of POH and multicomponent pattern represented the majority of POH on dermoscopy. Pigmentary and vascular patterns can be studied objectively with the help of dermoscopy. Dermoscopy is a handy , non invasive adjunct tool to study etiology and other associated factors which aids in diagnosis and management.
