Soft tissue sarcoma complicating neurofibromatosis type 1 during pregnancy

Abstract :

Neurofibromatosis type 1 (NF1) is the most common genodermatosis. This autosomal dominant disease predisposes to the development of benign tumors (neurofibromas) but also malignant tumors. Soft tissue sarcomas (STS) are rare tumors characterized by great anatomic, histological and prognostic heterogeneity. Genetic predisposition participates in 3% of the STS and corresponds essentially to NF1. Some studies have tried to establish the link between STS and pregnancy and have shown an increased risk of STS with late age at first pregnancy. We report the case of a 24-year-old patient with NF1 who developed a mass of the right flank during her first pregnancy, rapidly increasing in size, discovered in postpartum. Thoracoabdominopelvic CT showed in the right flank a parietal mass of heterogeneous density infiltrating the inferior vena cava, with close contact with the ipsilateral iliac bone and no other lesions at a distance. An exeterization of the tumor was performed witch histological examination showed an undifferentiated sarcomatous process with muscle infiltration and invasion of the bone cortex. A post-operative radio-chemiotherapy is planned. For this patient two questions are needed to be asked: Is this a sarcomatous transformation of plexiform, nodular neurofibromas, or a novo tumor? Is pregnancy considered us a risk factor for developing sarcomas? This new observation supports the need for careful and regular monitoring, especially during pregnancy, of patients with NF1.

Afaf Khouna has completed her medical studies at the age of 24 years from Mohammed first University, from faculty of Medicine and Pharmacy of Oujda, Morocco. In November 2016 she was admitted to the residency contest at the Faculty of Medicine and Pharmacy of Oujda: specialty "Dermatology, venereology and allergology". Inter University Diploma in "Paediatric Dermatology": obtained on 02/07/18 in Nantes University, France. She participates in the care of patients in the dermatology department and research activities. She has published couple of articles and abstracts in reputed journals and participates in many national and international scientific events.
