Purpose: Considering the increasing importance of evaluating and improving quality of life as one of the important determinants of therapeutic outcomes, the current study aimed to determine the validity and reliability of the Persian version of the European Quality of Life (EQ-5D-3L) questionnaire in Iranian patients with chronic non-specific low-back pain. Methods: After obtaining permission from the Euro QOL group, in order to assess the validity of the questionnaire, EQ- 5D-3L, SF-36, ODI, and FABQ were completed by 253 patients with chronic non-specific low-back pain. To examine construct validity, a hypothesis test was pre-defined, and discriminative validity was evaluated by the known-group method. Overall, 148 patients who did not report a change in their status recompleted the EQ-5D-3L questionnaire after a week, and test-retest reliability was determined. Results: Data analysis showed that the total score of EQ-5D- 3L questionnaire had a moderate to strong correlation with the total score of SF-36 scale, and EQ-5D-3L dimensions had a moderate to strong correlation with the related subscales of SF-36 and ODI (ρ=0.589 and ρ=0.47); also, a weak correlation was reported with the total score of FABQ (ρ= 0.190 and ρ= 0.125). The EQ-5D-3LIndex score could differentiate patients according to the severity of disease, based on the SF-36 scale. The intraclass correlation coefficients (ICCs) for EQ-5D-3LVAS and EQ-5D-3LIndex were 0.826 and 0.964, respectively. EQ- 5D-3LVAS also has a very good absolute reliability (SEM 1.673). Conclusion: The present study confirmed the validity and reliability of the Persian version of EQ-5D for measuring health-related quality of life in patients with chronic nonspecific low-back pain.
