Abstract :

The incidence of cutaneous tuberculosis is 0.1%. Lupus vulgaris (LV) is the most common, post-primary; highly destructive form of cutaneous tuberculosis in a patient with intact immunity. Various forms of LV seen are the plaque, ulcerative, hypertrophic, vegetative, tumor, papular and nodular forms. Objective: To report a rare case of multifocal LV-plaque and hypertrophic variant in the same patient. Case report: 11-yearold male presented with reddish-violet coloured scaly plaques on the left side of the face for the past 3 years and erythematous hyperkeratotic lesion over right elbow which appeared 6 months later. There was no history of the similar lesion in the family. No past history of pulmonary TB inpatient or family. Investigations- Mantoux test was positive (17mm x 20mm). Histopathology showed multiple granulomas in the dermis containing Langhans giant cells, epithelioid cells, lymphocytes and plasma cells with no caseation in the dermis. No acid-fast bacilli on zeil Neilson staining. Cultures were negative. Chest X-ray was normal. Treatment- the lesions resolved with the antitubercular treatment (Rifampicin, isoniazid, ethambutol and pyrazinamide) which was continued for 6 months. Conclusion- As LV is a paucibacillary form of tuberculosis, demonstration of the organism (Mycobacterium Tuberculosis) is not possible. Considering the clinical scenario and histopathology, due to high index of suspicion in an endemic country like India, antitubercular treatment should be started and continued if the lesions respond.

Amrit Kaur has completed her MBBS at the age of 24 years from Government Medical College And Hospital, Chandigarh (UT), INDIA. She is currently pursuing her post graduation junior residency i.e. MD dermatology, venereology and leprosy from Government Medical College And Hospital, Chandigarh (UT), INDIA. She has 2 case reports that are under publications. She has presented around 5 posters and 2 oral presentations in different national conferences.
