A 59-year-old male, known case of Ischemic cardiomyopathy, Chronic liver disease and Hypertension, presented with complaints of multiple red raised itchy oozy lesions over the trunk, face & extremities for last 3 years. On examination he had multiple erythematous oozy crusted lesions over trunk and macerated oozy plaques over flexures. Histopathology revealed sheets of atypical histiocytic at the dermo-epidermal junction extending into dermis. Immunohistochemistry showed CD1a, Langerin positive. PET scan revealed bilateral pleural effusion, pericardial effusion and bilateral axillary lymph nodes. Based on evaluation, he was diagnosed as adult-onset Langerhans cell histiocytosis with multisystem involvement. He was managed with Carabine with favorable response.Langerhans cell Histiocytosis is common in 2-5 years of age and most of the cases are <15 years of age: 2-5 cases per million. LCH in adults is extremely rare with only 1-2 cases per million. We herein describe a rare case which was successfully managed with topical steroids and chemotherapeutic agent and discuss its manifestations for its early detection and management.
