Hospital readmissions have a significant impact on the healthcare system. The Centres of Medicare and Medicaid Services (CMS) continue to expand their criteria of disease states that will be penalized for readmissions. Transitional care involves 3 domains of transfer: information, education, and destination. Medicare suggests that a 19% all-cause 30-day readmission rate, of which 47% may be preventable. Additionally, approximately 65% of adverse drug events (ADE) that occur during post hospital discharge are medication-related. Pharmacists can play a large role in the development of transitions of care (TOC) program. In current literature, pharmacist involvement has been shown to decrease ADEs and increase patient satisfaction. In this session, attendees will learn the transitions of care program to achieve goals to help decrease readmission rates at North Florida Regional Medical Center. Description of Proposal: During this session, the presenters will provide details on North Florida Regional�?¢�?�?�?�?s TOC program with the primary goal to decrease 30-day readmission rates. The other goal of the program includes tracking metrics of daily pharmacy services activity and its impact on patient care. The pharmacy department is working in conjunction with an institutional Readmissions Committee to continue to improve the program. The program details from its inception as a bedside pharmacy medication counseling program and monthly reports of pharmacy interventions completed will be shared with the attendees.
