Specialist directed remote heart management in the post-acute facility setting improves medication management and clinical outcomes

Abstract :

Although much time and resources are devoted to improving re-admission strategies for congestive heart failure in the outpatient setting, there is a paucity of effort directed towards post-acute facilities such as nursing homes and assisted living facilities. The vast majority of management of chronic cardiovascular diseases in this setting is performed by primary care physicians, nurse practitioners and physician assistants with little or no specialty involvement. The result is variable medication management with a high admission and re-admission rate of patients with chronic cardiovascular disease. Given the looming shortage of cardiovascular specialists in the United States, alternatives strategies to specialty care in this setting are needed. We observed the use of Pulsario remote heart management system in this setting. This system enables real time specialty care to be delivered without the physical presence of the cardiovascular specialist. Within the first 30 days of system deployment, there was an improvement in guideline directed medical therapy specific to a patient�??s underlying medical condition by 92.3%, the management of hypertension to goal was improved by 77.8% and 13.6% of patients avoided hospitalization. These results reveal the rapid improvements in medical therapy, reduction in hospitalization and feasibility of specialty care delivery by Pulsario remote heart management system within the post-acute care facility.
