Abstract :

Identification of genetic biomarkers that define a specific pharmacogenetic phenotype presents a complex problem of Pharmacogenomics. Several approaches have been found useful to select genetic markers which accurately predict drug response or adverse drug reactions. The general strategy includes the search for statistical association between SNPs and the phenotype. Using a prokinetic drug domperidone as an example, the major approaches to identification of SNPs important for effective and safe use of this drug will be outlined. Analysis of candidate genes presumed to contribute to domperidone pharmacotherapeutic effects, along with genome-wide genotyping helped to generate lists of SNPs associated with domperidone effects. Hard biological evidence is required to identify the causal genetic polymorphisms. The limitations and prospects of the pharmacogenetic analysis will be discussed. The combination of gene-based diagnostics, biological evidence, and data mining has the potential to make Pharmacogenomics an efficient tool of practical medicine.
