Role of cell block technique in improving diagnostic cytopathology

Abstract :

One of the constraints of the conventional fine needle aspiration (FNA) smear is the limited material available for adjuvant diagnostic investigations. The use of the cell block technique enables the retrieval of small tissue fragments in a fluid specimen which are processed to form a paraffin block. It has been widely accepted that this method of analysis increases the cellular yield and improves diagnostic accuracy. We have used cell block for various systems like for breast and cervix. both of them were two year studies. Breast and Cervical cancer are the2nd and 4th most common in third world countries like India. Breast study of 60 cases included comparison between FNAC, cell block, manual liquid based cytology (MLBC)and confirmation by histopathology Cervical cases had 60 cases with comparison of conventional pap smear(CPS), MLBC, CELL BLOCK, HPV with histopathology. The technique of cell block preparation in our set up is alcohol 60% with 10% formalin used as 60:40 ratios for fixation of material. Uses of cell block are diagnosis in haemorrhoid aspirates, and ihc on the cell block material. IHC panel we have used ER, PR, HER2NEU, and KI67 for breast cancer panel. P16INKA and KI67 for cervical cancer detection have been used as an ancillary technique for MLBC in cervix. Cell block is an efficient and easy to follow technique for improving the diagnostic accuracy of premalignant and malignant cases which can be used as an adjunct and in difficult to operate cases.

Dr Nandini N. M, Professor, JSS Medical College, Mysore, is attached as a teaching faculty from the past 25 years in the department of pathology . She has worked in the field of cytology of cervix and breast. She has attended many national and international conferences in countries like U.K, USA, Singapore and Netherland. She has worked on liquid based cytology, cell block technique and has come up with indegenious methods. She has severeal publications and books to her credit written on cervical and breast cancer.
