Abstract :

Global evidence demonstrates the safety and feasibility of the robotic approach for gynaecologic oncology surgery and lower conversion rate to laparotomy. Robotic surgery is the latest development in minimal invasive surgery in gynaecology. With many technological advantages, it allows the surgeon better vision, autonomy, surgical dexterity, precision and control of the surgical field. For staging of endometrial cancer robot-assisted surgery may be the most effective, least invasive treatment option. Through tiny incisions we can operate with greater precision and control, better surgical dissection, particularly for lymph nodes. It enables us to perform surgical staging in morbidly obese patient and minimize the pain and risk associated with large incisions while increasing the likelihood of a fast recovery and excellent clinical outcomes. Other advantages of the robotic approach include threedimensional imaging, reduced blood loss, and shortened hospital stay and recovery time. In my presentation I will describe our experience and outcome of robot assisted surgical staging for 36 cases of endometrial carcinoma. There was ease of operation because of better visualization and range of motion of robotic instruments and there were no per-operative or post-operative complications. Conversion rate was zero. However, higher cost of surgery continues to limit a wider usage. We believe that robot assisted surgical staging for cancer endometrium is a feasible and a good option that should be used whenever available. However, further prospective studies and late follow-up results are needed to fully assess the value of this new technology.

Sabhyata Gupta is a Gynaecologist with 27 years of post MD experience. She is the Director and Head of Gynaecology and Gynaecologic Oncology at a Tertiary End Premier Hospital Medanta The Medicity, Gurugram, India. She is the first Gynaecologist from India to do Robotic Surgery for benign and malignant gynaecological disorders. She has contributed to various text books, national and international journals, delivered lectures and presented personal work at various national/international conferences. She has started the department in three start up hospitals in India. Her field of interests include: Teaching robotic surgery, Laparoscopic surgery and Gynaecologic Oncology. She is Vice President, Fertility Preservation Society of India for the period 2014–2018.
