Abstract :

Background: We are describing an extremely rare case of Chiari?s network with ventricular arrhythmias. It is the only case with bigemny rhythm and ventricular tachycardia (VT) in a neonate with Chiari?s network described in the literature. Case presentation: This is a late preterm (35 weeks), with intrauterine growth restriction (IUGR) that developed uniform PVCs as often as in bigeminy, on day 5 of life. On day 18 of life, she developed ventricular tachycardia VT which responded (converted to normal sinus rhythm) to Adenosine boluses. (The on-call team thought it was Supraventricular Tachycardia) Echocardiogram showed Chiari?s network. This network subsequently vanished on further follow up. Discussion: Chiari?s network is a congenital remnant of incomplete reabsorption of the right valve of the sinus venosus. Chiari?s network is often clinically insignificant. We think our case further support Dominguez EF et al. postulation that insufficient absorption of Chiari?s network may occur in conjunction with abnormalities of the cardiac conduction and cardiac rhythm. This case further emphasizes adenosine as a possible effective treatment in terminating VT in neonates. It is important for pediatricians to be aware of this condition because although it is generally considered a benign normal variation, it might lead to significant morbidities.

Bassel Mohammad Nijres had his medical school in Syria. He is an Arab Board Certified in pediatrics. Currently he is a PGY III in pediatrics at Woodhull Hospital. He will start fellowship in pediatric cardiology at Rush University on 07/2015.
