Rapid restoration of coronary blood flow remains the cornerstone of treatment for patients presenting with ST-elevation myocardial infarction (STEMI). Over the past two decades, major advances in primary percutaneous coronary intervention (PCI), coronary imaging, pharmacotherapy, and emergency cardiovascular care have substantially reduced mortality and improved long-term outcomes. This presentation reviews current evidence supporting timely reperfusion therapy and dis-cusses recent innovations in interventional cardiology that continue to optimize patient care.
The presentation examines advances in intracoronary imaging, physiological lesion assessment, drug-eluting stent technol-ogy, mechanical circulatory support devices, and individualized antithrombotic therapy. Particular emphasis will be placed on improving systems of care that reduce treatment delays, facilitate rapid diagnosis, and ensure timely access to specialized cardiovascular centers.
Recent international clinical trials evaluating complete revascularization, personalized pharmacological therapy, and post-myocardial infarction rehabilitation will also be reviewed. Future developments include artificial intelligence-assisted emergency diagnosis, robotic-assisted coronary intervention, and precision cardiovascular medicine designed to further improve survival and quality of life following acute myocardial infarction.
Petr Widimský is Professor of Cardiology at the Third Faculty of Medicine, Charles University, Prague, Czech Republic, and Head of the Cardiology Department at University Hospital Královské Vinohrady. He is internationally recognized for his con-tributions to interventional cardiology, acute myocardial infarction management, and primary percutaneous coronary inter-vention. His research has helped establish modern reperfusion strategies adopted in cardiovascular guidelines worldwide.
