Abstract :

Introduction: Breast cancer is the most common malignancy in women worldwide. In the UK, breast cancer accounts for 15% of all new cancer cases (2016-2018). In addition, breast cancer is the 4th most common cause of cancer death in the UK, accounting for 7% of all cancer deaths (2018), and the 2nd most common cause of cancer death in females. Despite the recent advances in surgery, many patients still require mastectomy for better control of the tumor which is associated with many morbidities including psychic trauma. The aim of this project is to investigate if we offer patients opted for mastectomy the options of reconstruction, either immediate or delayed reconstruction during discussing the surgical management plan according to the NICE guidelines. Patients and Methods: A retrospective study was done on a sample of 19 patients over a time period of 4 months. Data were collected using case note review, e-notes computer search & data collection form Results: 5 patients of 19 only were offered reconstruction during the preoperative consultation before mastectomy. 14 patients did have a discussion regarding reconstruction. Of the 14 patients, 6 of them had multiple comorbidities in addition to advanced age which ruled out reconstructive surgery according to the NICE guidelines. Additional 8 patients, had not any discussion regarding reconstruction. All these patients had their surgery during the COVID-19 pandemic which affected the surgical management and limited any discussion regarding reconstructive surgery. Most of the patients who were generally fit, have been offered reconstruction surgery; however, not all of them agreed for reconstruction. Conclusion: We should offer the options of reconstructions (either immediate or delayed) for every patient opted for mastectomy unless there are severe comorbidities that can affect the outcome of the surgery according to NICE guidelines.
