Uterine cancer is the sixth most reported cancer in women worldwide and it is one of the first malignancies attributable to obesity. The relationship of obesity and endometrial cancer is stronger for endometrial cancer than for any other type of cancer. Compared to women who are a healthy weight, endometrial cancer is twice as common in overweight women (BMI 25 to 29.9) and more than three times as common in obese women (BMI 30 or higher) according to the American Cancer Society. Obesity is responsible for more than half of all endometrial malignancies. Endometrioid (type 1) endometrial cancer is more commonly associated with obesity than type 2 (non-endometrioid cancers such as serous), however both subtypes show an increased risk with obesity. For decades, the link between obesity, metabolic syndrome, and endometrial cancer has been known and understood. Furthermore, studies show that obesity as a cancer risk factor is not adequately addressed by gynecologic oncologists and therefore they fail to provide appropriate weight loss and lifestyle modification counselling during endometrial cancer screening, diagnosis, and follow-up. The goal of this article is to examine how obesity impacts endometrial cancer morbidity and mortality, as well as to suggest therapy options to reduce these factors.
