Abstract:

Why go extensive when conservation is an option. Periapical lesions of endodontic origin are mostly produced by an inflammatory response at the root apices of teeth with non-vital pulps. It is caused by imbalance between microbial factors and host defences at the interface between the infected radicular pulp and periodontal ligament that results in local inflammation, resorption of hard tissues, and destruction of other periapical tissues. Surgery is considered to be necessary for treating periapical endodontic lesions, particularly when they are large in size. It is still sometimes preferred when the resolution of periapical lesion is delayed. All inflammatory periapical lesions should be initially treated with conservative non-surgical procedures.
