Statement of the Problem: The thumb represents 40-50% of hand function. For the degloving injury and avulsed amputation of the thumb, it???s vital to recover the length, appearance, sensibility of the thumb for the entire function and aesthetics of hand. For those severe cases which have no possibility to be replanted, there are a variety of repair methods have been applied. Each method has its own advantages and disadvantages. A wrap-around skin and nail bed flap from the great toe could provide a satisfied appearance. But the high expertise in microvascular surgery, postoperative care and the high operation cost of this challenging microsurgery limit the application of it in the most primary hospital and poor area. On the other hand, some patients refuse to utilize the tissues of the foot for some psychological or religious reasons. The purpose of this study is to describe a kind of composite regional island flap has a less technological requirement, operation time and cost. Methodology and Theoretical Orientation: A set of bilobed homodigital neurovascular pedicled island flaps with a common pedicle or one homodigital island flap combined with a dorsoradial collateral artery flap were harvested and transferred to cover the palmar and dorsal side of the injured thumb respectively. The donor sites were covered by a skin graft. Findings: All of the flaps and grafted skins were survived. The length, mobility, sensation and durable performance of the thumb were obtained. And the patients were satisfied with the aesthetics of their hands. Conclusion and Significance: Composite regional island flap is a safe, convenient and economic reconstruction option for the degloving injury and avulsed amputation of the thumb.
