Abstract:

Infant acute leukemia (IAL) has an unique profile characterized by the high incidence of translocations involving the MLL gene located at the 11q 23 region. To test the potential role of intrauterine and perinatal factors linked to the risk of IAL development, a hospital based-case-control study was conducted in different cities of Brazil. A total of 252 children 0-23 months with newly diagnosed IAL during 1999-2007 and 440 age-matched controls selected from hospitals in the same cities were enrolled. Several maternal and paternal exposures to selected environmental risk factors 3 months before pregnancy, during pregnancy, and 3 months after birth were obtained by interview. A statistically significant association between maternal use of hormones during pregnancy and IAL was observed in a multivariate analysis, adjusted odds ratio, OR=8.76, 95% C.I. 2.85-26.93. According to birth weight distribution, cases born 4000 g or more showed an OR=2.28, 95% C.I. 1.08-4.75 comparatively to those born 2500-2.999 g in the general population. Relatively to pesticides exposure, an adjusted OR=2.09, 95% C.I. 1.39-3.13 was observed between acute lymphocytic leukemia (ALL) and maternal exposure to pesticides, being higher for acute myeloid leukemia, AML, with and adjusted OR=3.23, 95% C.I. 1.74-5.99). An adjusted OR=1.78 (95% C.I. 1.13-2.81) was observed between hair dyes and hair straightening cosmetics maternal use during the 1st trimester of pregnancy and ALL. Maternal heavy smoking (>20 cigarettes daily) during pregnancy revealed an OR=5.28, 95% C.I. 1.22-45.5 for ALL, and an OR=78, 95% 1.33-45.5 during breastfeeding.
