Acantholytic squamous cell carcinoma of the maxilla: report of a case

Abstract:

Squamous cell carcinoma (SCC) is the most common malignancy of the oral cavity, and there were some variants of it. Acantholytic squamous cell carcinoma (ASCC) is an uncommon histopathologic variant of squamous cell carcinoma, and it is also known as adenoid SCC, pseudoglandular SCC, SCC with gland-like features, angiosarcoma-like SCC, pseudovascular adenoid SCC. It occurs commonly in sun-exposed areas, and oral cavity lesion is very rare. Here we report a recurred case of ASCC in the maxilla with CT and MR images. A 59-year-old female patient visited department of oral medicine with her symptoms of pain and burning sensation in the maxilla. Irregular hypertrophic lesion with verrucous surface of erythema and ulcer was shown on her palate and edentulous gingiva of right maxilla. Incisional biopsy was done and the result was reported as SCC. CT and MR images revealed enhancing mass in the right palate. There was no evidence of lymph node metastasis and perineural spread. The final diagnosis concluded from the whole mass was ASCC. After the surgery and radiotherapy, extensive local recurrence including destruction of cranial base and intracranial invasion was detected after 3 months from the surgery. ASCC is rare variant of SCC, and oral ASCC is extremely rare and unknown. Though prognosis is still controversial, the reported cases including this case in recent days had weight on poor prognosis. Careful work-up, earlier and radical treatment, and close follow-up with imaging should be emphasized in ASCC

Jo-Eun Kim is an Assistance Professor of Department of Oral and Maxillofacial Radiology and Dental Research Institute, School of Dentistry, Seoul National University, South Korea. Soon-chul Choi has completed his PhD from Seoul National University and he has been a professor at Department of Oral and Maxillofacial Radiology of Seoul National University since 1989. He served as the Dean of the Dental College for two years from 2010.
