Effect of comprehensive prosthodontic rehabilitation on the Quality of life of patients with temporomandibular Joint dysfunction in need of a full mouth reconstruction

Abstract objective: Evaluation of the effectiveness of various approaches to prosthodontic rehabilitation of edentulous patients with temporomandibular joint dysfunctions based on their life quality indicators. Methods: We carried out a comprehensive examination an d treatment of 647 edentulous patients in need of a full mouth reconstruction. The patients were divided into 3 groups: patients in group 1 (n = 218) were treated with standard methods of prosthodontic dentistry; patients in group 2 (n = 195) were subjected to certain elements of the interdisciplinary approach to prosthodontic rehabilitation; patients in group 3 (n = 234) were fully subjected to the interdisciplinary approach to prosthodontic rehabilitation, taking into account the patient’s individual anatomical and physiological characteristics. The follow-up of the patients included in the study was carried out for 7 years. The key stage in the treatment of patients of group 3 was a complete and consistent examination of patients with procedures supplemented by the detection of the temporomandibular joint dysfunctions. The causal relationships of these dysfunctions with malocclusion were identified in order to plan the prosthodontic treatment. Dental quality of life of patients was assessed using OHIP-14 questionnaire. Results: It was found that the improvement of the condition of the temporomandibular joint when the interdisciplinary approach to prosthodontic rehabilitation is used is manifested by statistically significant (relative to the characteristics of comparison groups) changes in indicators: decrease in the patterns of pain in the joint area according to a visual analog scale, more pronounced (in comparison with the group of patients who were subjected to a standard complex of treatment and rehabilitation procedures) decrease in all scores of OHIP-14 questionnaire, indicating an improvement in the patients’ quality of life. Conclusions: Application of the interdisciplinary approach in the course of prosthodontic rehabilitation facilitates pronounced and persistent improvement in the indicators of the dental quality of life of edentulous patients in need of a full mouth reconstruction.
